UHSAA Pass Request Form
Picture ID must be shown when pass is used. The allotment of passes as mandated by the Board of Trustees is 20, the first pass is complimentary. Additional passes for Assistant Superintendents, District Athletic Directors, and Board of Education members may be purchased for $35 each (please do not list anyone who is serving as a UHSAA Board of Trustees member as those will be issued directly from our office).
PLEASE LIST ONLY THOSE PEOPLE WHO QUALIFY. ALL POSITIONS MUST BE INDICATED ON THE FORM OR PASSES WILL NOT BE SENT.
Exchanging a pass for a different name will only be allowed until October 1. After that time, a new pass will need to be purchased and will count toward your allotment.
Please do not order passes for Silver Pass holders. You can request the silver pass application form from Jennifer Thomas at jthomas@uhsaa.org.
Press submit when done and an invoice will be sent to your district to collect the total amount owed.
